The CP@clinic program offers you a free health check by a community paramedic trained to assist people with managing their health outside the emergency setting. Community paramedics use their expertise, knowledge and skills to conduct health assessments and provide health advice. They can help you improve your health.
At a CP@clinic, a community paramedic will:
- Ask some questions about your health;
- Review your medications;
- Take your blood pressure;
- Ask you to do a walking test;
- Give you health advice and information;
- Set health goals with you; and,
- Connect you with your primary care provider and other community resources.
All personal information is kept private and confidential. It will not be shared with anyone or any community services without your consent.
