Referral and triage

Referrals are received fortnightly from the Department of Families, Fairness and Housing (DFFH).

The PTGH Nurse Coordinator reviews and collects referral health information as part of the triage process. Not all children referred will require a multi-disciplinary assessment. The PTGH Nurse Coordinator may support the child and their carers in other ways, such as re-engaging with a previously seen paediatrician or a maternal and child health nurse.

Assessment team

Assessments are conducted at the PTGH Clinic located at 155 High Street, Wodonga.

Participants may be assessed by:

  • Paediatrician
  • Psychologist
  • Speech Pathologist
  • Occupational Therapist

Outcomes and follow-up

A Health Summary or Health Management Plan is produced to document the participant’s health information and assessment. The PTGH team will also provide any necessary referrals to DFFH and the carer or parent.
