Seniors Lifestyle Victoria offers personalised support for older people who may be finding it harder to manage everyday activities, stay active, eat well or stay socially connected.
The program is not one-size-fits-all. A Gateway Health coordinator works with each person to understand their goals and connect them with local supports.
Support may include:
- Physical activity and strength-based support
- Nutrition support
- Medication management
- Social connection and wellbeing
- Help navigating local health and community services
Who can access the program?
Eligibility criteria apply, but the program is generally for people who are:
- Aged 65 years and over, or 50 years and over for Aboriginal and Torres Strait Islander people
- Living at home or in the community
- Experiencing mild to moderate changes in health or everyday function
- Wanting support to stay active, independent and well
How the program works
Participants will work with a Gateway Health coordinator to talk through their goals and create a personalised action plan.
This may include support to improve strength, balance, nutrition, confidence, social connection or access to other local services.
