We are currently seeking an enthusiastic, experienced Social Worker to join the Rural Health Team in this new role. This exciting role reports to the Rural Health Team Manager, and will provide a timely and flexible services to eligible clients under the Home and Community Care Program for Young People (HACC–PYP). HACC-PYP provides basic support and maintenance services for people aged under 65 and Aboriginal people aged under 50 and their carers to help them remain independent at home and in the community. Our service is predominately a home-based service.
This role is part of a multidisciplinary team to provide services to people (primarily 18 – 65 years) with disabilities or chronic conditions impacting on their daily function.
